Billing
and Referral Policy
Medical insurances change frequently and many
managed care plans require a referral from a primary care physician.
Urologic Surgery Associates participates with the overwhelming
majority of insurance plans. However, it is wise to call ahead
to verify participation with the less common carriers and to
assure coverage for certain specialized services. It is the
patient’s responsibility to obtain all necessary referrals
prior to a scheduled appointment. If a referral cannot be obtained,
then insurance billing may be denied in which case patient payment
will be expected for services. For patients with appropriate
coverage, charges will be submitted directly to your primary
insurance carrier. Additional balances including coinsurance
and/or copayment will be billed to the patient unless secondary
insurance information is also provided. Self pay patients are
expected to pay at the time services are rendered unless an
alternative payment plan has been approved. Our billing department
can be reached to answer any detailed questions at 410-296-5333.
